Health Services
Prior to the beginning of school, please notify the school nurse of any health issues such as heart problems, diabetes, severe allergies, hearing loss, chronic pain, asthma, seizure disorders, etc. This enables the school to make necessary accommodations to ensure that students are safe. The school nurse is also in charge of mandated testing which includes scoliosis, vision, and hearing screening.

Please keep information on this card current. Students cannot be
dismissed from school due to illness unless a parent/guardian or
designated person is notified. In case of emergency when parents cannot
be reached, 911 or the designated physician will be called and
necessary treatment will be administered.
Medication
Medication
The
Board of Trustees recognizes that students may need to take prescribed
medication during the school day in order to be able to attend school
without jeopardizing their health. When the District has received
written statements from the student’s physician and parent/guardian,
designated personnel shall assist the student in taking the medication.
In addition, upon written request, designated personnel may assist the
student in monitoring, testing or other treatment of an existing medical
condition.
- All medication, including non-prescription drugs, requires a signed release from parents and the attending physician in order for school personnel to administer the medicine on campus. Each prescription must be accompanied by a written prescription and statement or note from the attending physician. Release forms may be obtained from the school office.
- Students are not allowed to carry any form of medication (prescription or over-the-counter) at any time while on campus except those medications that a physician requests the pupil be allowed to carry [e.g. inhalant for asthma, Epinephrine, Auto-injector (Epi-Pen) for severe bee-sting allergies]!! Any medication must be placed under lock and key in the nurse’s office in the administration building for safety of all students! A complete copy of Board Policy and Administrative Regulation 5141.21 is available in the school office.
Immunization
Immunization
State law requires that all incoming seventh grade students have a
3-dose series of Hepatitis B vaccine and a second dose of measles
containing vaccine (MMR). A tetanus-diphtheria booster is recommended.
